FORMS/DOCUMENTATION
All band students must complete and submit these forms to the band director for each school year.
Physical copies of these forms are also available in the band room.

1) THS Band Personnel Form
2) Medical Form (must be signed AND NOTARIZED) - 2025 form not yet released
3) Physical Form (AHSAA Physical Form) - valid for 2025

Theodore High School provides instruments for Tuba, Baritone, French Horn, and Percussion students (most other instruments are owned/rented by the student). If your child is in need of a quality instrument for rent or purchase, please contact Andy's Music Store for more information!
